It seems like self test is failing 
See the attached picture: I also made a short movie about what the drive does during the self test, without cover. It's available from here:
Furthermore, it's not listing the MD content anymore - further reduction of functionality. In your practice is this a problem of the disk, or the drive?
Can you recommend a next step?
